About us

We are an award-winning non-profit nursing home providing resident-centered long-term care and short-term rehab services. We accept Medicare, Medicaid, Private Insurance, and Private Pay. We employ approximately 135 team members to help us succeed in providing the best possible quality of care and enhancing each individual's quality of life. Since 2002 we have been in the process of transforming our organization from an institutional model of care which has been common for more than thirty-years to a 'resident-centered' care model promoting resident choice, resident schedule, and resident first.

  • We are thrilled to share some exciting updates regarding our new campus project! Back in March 2023, thanks to the unwavering support of the Johnson County Board of County Commissioners, Evergreen Community embarked on a journey to construct a vibrant new community. We recognized the need to evolve from our current setup of semi-private rooms and 112 beds, which no longer aligns with the demands of the community. Fast forward to April 4, 2024, and we are proud to announce that Evergreen has secured land under contract in Olathe, marking a significant milestone in our journey. The Olathe Planning Department, Olathe Planning Commission, and Olathe City Council have enthusiastically approved our building project, paving the way for our vision to become a reality. Additionally, we are excited to partner with Midland PACE, who will be developing alongside us to better serve the needs of the Medicaid-eligible elderly population in the county. Our collaboration with the City of Olathe has been instrumental in expediting the approval of all necessary rezoning, site plan approvals, and building permits. We are thrilled to report the approval of our site development plans and architectural plans. Evergreen residents and staff were instrumental in helping design and revise the architectural design of the building. With construction plans well underway, we anticipate the completion of our new campus in early 2026. Upon completion, our new campus will offer an array of services tailored to meet the diverse needs of our community. From skilled nursing care in all private rooms to assisted living memory care, independent living apartments, and hospice services, our goal is to provide comprehensive care and support to all those we serve. Furthermore, we are excited to announce that Evergreen will be offering meals to PACE participants, along with a variety of life enrichment activities and chapel services. Additionally, we will be collaborating with PACE through various clinics, including dental, podiatry, and behavioral health services. As we continue to make strides toward the completion of our new campus, we recognize the need for additional financial support to ensure its success. Therefore, we are excited to announce the launch of our capital campaign aimed at securing the necessary funds to make our vision a reality.   • Help us celebrate the newest Evergreen Hero of the Month, Afton Sanchez, Laundry/Community Care Partner   These are the things the team said about Afton: Afton is very patient and kind and she is a hard worker! Afton helped a resident in distress who thought her clothes and shoes were missing. Afton walked her through the laundry room and put the resident’s mind at ease.
